PCN Staff

We have Additional Roles Staff who are shared between the GPNET5 Primary Care Network.

They include First Contact Practitioners, Paramedics, Care Coordinators, Social Prescribers, Pharmacists and Mental Health Nurses  - these staff support the PCN and Practice Team.

First Contact Practitioner

What is a First Contact Practitioner (FCP)?

An experienced physiotherapist who has the advanced skills necessary to assess, diagnose and recommend appropriate treatment for problems that affect your neck, back, joints or muscles. They act as your first point of contact and offer advice and referrals for further treatment and scans.

Around one in five people book in to see their doctor with a neck, back, joint or muscle problem, and often have to wait for a GP referral to get access to physiotherapy advice for their conditions. Seeing an FCP gives you the opportunity to see the right person at the right time. The expertise of the FCP means that you will have quicker access to the treatment you need.

Paramedic

Our paramedic can carry out Acute Home Visits as well as assisting the Practice team carrying out annual reviews and checks on our housebound population.

Care Coordinators

The care coordinators assist the Practice admin team with a whole ranges of tasks incuding vaccination booking, care planning and audits.

Social Prescriber

A Social Prescriber is a health professional who works with your GP Practice to help you improve your health and wellbeing. By working closely with your GP and other healthcare professionals, they can provide you with a helping hand to achieve your health and social needs.

It starts with a conversation with your doctor or another person in the practice team, who then refers you to a social prescribing link worker. If you would like to book an appointment with your social prescribing link worker, ask your GP or speak to a member of the practice staff.

General Practice Assistant

Our GP assistant assists the GPs with clinical administration. 

Pharmacists

Clinical pharmacists work as part of the general practice team to improve value and outcomes from medicines and consult with and treat patients directly. This includes providing extra help to manage long-term conditions, advice for those on multiple medicines and better access to health checks. The role is pivotal to improving the quality of care and ensuring patient safety.
